Building Use Request

Any extra event and/or gathering using FBC facilities needs to have a building use form.

  • This maintains the calendar and scheduling
  • Arranges proper facilities management

Item Check Out

We have lots of items that we keep on site and we would love for you to be able to use them. In order to help keep track of these items we ask that you fill out a check out form.

Yearly Permission Form

We require a yearly permission form on file for any person under the age of 18 to travel off site with First Baptist Church. Insurance card must also be uploaded.
